I had some of the same issues that others were sharing. I tried reflashing the firmware and performing the factory reset. It turns out that the factory reset doesn't reset everything. Thankfully I had a data.bin file from when I first received the controller. I have attached it below. This will factory reset your controller and likely solve many of the issues you are facing with button mappings and features not working.
